We have created the Sauk County Health Care Center Foundation, a completely volunteer organization, to collect donations and provide resident amenities for the Health Care Center. For as little as $5, you can make a difference in the life of a Center resident by contributing to music, art, entertainment, nature, and more. Monetary gifts of all sizes are welcome. You can even designate how your contribution will be used by gifting to specific programs such as providing lawn benches or outing programs. Giving is easy and completely tax deductible. You may use our secure online payment system, or simply mail in the back portion of this pamphlet with your check.
For larger donations, naming rights and plaques are available. Contact the administrator at the Sauk County Health Care Foundation for more information. Gifts in memory of former residents will have a special place of recognition. All donations from persons and organizations will be publicly thanked; anonymous giving is also available.
