With your generous support, Sauk County will open a new health care facility on the western edge of Reedsburg in late 2009. This $15 million,
82-bed skilled nursing center will be an environmentally friendly, energy efficient building offering short and long-term care for residents who
need rehabilitation, pain management and memory support care. The new center is a welcome replacement for the original facility that
successfully served our community since 1871. Despite a number of additions and updates over the years, the old facility was simply unable to
accommodate modern health care needs, was inefficient and very expensive to maintain. While the new center will be a state-of-the-art building
that meets the residents’ medical, physical and emotional needs, tight budget constraints prohibited a number of desirable amenities from being
funded. Garden areas, patio furniture, wall decorations, artwork, social outings and enhancements in the common rooms are just a few of the
unfunded amenities. These are the extras that could dramatically improve the quality of life for our residents — and provide them with a
comfortable home away from home.

Now there is a way that you can help!


We have created the Sauk County Health Care Center Foundation, a completely volunteer organization established to collect donations and
provide resident amenities for the Center. For as little as $5, you can make a difference in the life of a Center resident by contributing to music,
art, entertainment, nature, and more. Monetary gifts of all sizes are welcome. You can even designate how your contribution will be used by
gifting to specific programs such as providing lawn benches or outing programs. Giving is easy and completely tax deductible. You may use our
secure online payment system, or simply mail in the back portion of this pamphlet with your check.

information. Gifts in memory of former residents will have a special place of recognition. All donations from persons and organizations will be
publicly thanked; anonymous giving is also available.
